A difficult management case of severe hepatotoxicity with Ribociclib

Atualmente, a combinação de hormonoterapia com inibidores de ciclina-cínases 4 e 6 (iCDK4/6) é o tratamento de primeira linha de cancro da mama metastático com recetores hormonais positivos e HER2 negativo. A seleção entre os diferentes iCDK4/6 é um desafio tendo em conta a escassez de estudos comparativos de eficácia e aos diferentes perfis de toxicidade. A toxicidade hepatica é comum e frequentemente manuseável. Neste artigo, relatamos o caso de uma doente com cancro de mama metastático que apresentou hepatotoxicidade grave e persistente com o tratamento com ribociclib e letrozol. A mudança para palbociclib, associado a menor toxicidade hepática, permitiu uma melhor tolerância ao tratamento. Neste caso, verificou-se eficácia sustentada com o tratamento com Palbociclib, realçando a importância da seleção criteriosa de inibidores de CDK4/6 e o seu uso sequential em caso de toxicidade.
